If you are banned you can go to ban appeals then it will record or see by admin, if you are prison it will not recorded.

Only If an admin bans your account It includes your IP address, and no prison record go on your IP address or whatsoever. Yes, there is a possibility that your forum account will be banned as well depending on the decision given by the admin who banned you.

Admins can check logs if your account/IP is banned or was banned. You can sumbit a ban appeal in order to get unbanned.

Ban record - IP.

They check your IP randomly. Nothing's related for forums, except if you were forums banned too.
