1-) Copy and save your answers of each question you've wrote in the form,
2-) Make a DoC application and fill it with a word '' Bug issue '' and paste your application,
3-) Once done, click on '' Edit '' and paste each answer of the question you saved above.

Seems formats or applications are kinda bugged as I've been hearing this issue from many of them by now.
